Story and some of the bases can get bugged. If you didn't get that Janemba sword, then you will never get 100% unless they release it later. So that made me see that the prizes given through the weekly raids DO count for equipment completion.
Then I decided to send it all to hell and bought the missing items with the shop mod, those that were not in PQs or anywhere else and got 100% to equip that way.
